Waiver to do over 14 cp/semester (1 Viewer)

Hey guys,

I've decided to do an extra unit next semester, the unit has been approved on eStudent (it was one of those that needs permission of the dean) but it will take me over 14cp.
At the beginning of the year everyone had their waivers signed but now I'm confused as to which application form to fill out and who to give it to?

Thanks for that- couldn't find the right form anywhere.
It's not a LAW unit, its RSN so will I give it to the receptionist in the W6A building then?
