with my hsc exams alone, is it the rank of your mark compared to the cohort or the mark you get from the exam, that determines my atar?

Both. The first thing you said determines assessments mark, the 2nd thing determines the exam mark. Together these are added and divided by 2 giving a HSC mark. If you get a decimal such as 90.5 it gets rounded up to 91
