How often are questions "repeated".

How often are there "new" questions which have never been seen before.

Are the questions somewhat predictable?

Questions are never repeated, but can come in the same form. After doing a lot of past papers, you begin to realise the patterns of questions and can complete questions faster as you recognise the best methods to do them while others are still exploring them.

So questions in the 4 unit math paper rarely have new question formats?

For example, a question could be 1+1=2
But a "new" question could be:
Andy has one apple and John has another apple. How much apples do they both have?

Some questions do get asked very often, e.g. things like asking to derive the equation of the tangent to an ellipse at an arbitrary given point.

Not sure about 4U specifically
but easier questions will be fairly stock standard, with different questions but applying the same methods
to use your analogy (hopefully it was an analogy lol)

i.e 1+1 = 2, 30 + 45 = 75 or 8952+4712=13664

harder questions will require you to use the skills in unique ways, or combine them in atypical ways
I think what is special about 4U is it requires a certain level of intuition to do harder questions, something you develop with time
