Documenting the major programming project

Hi,

I m doing documentation for my major project in VB and I am looking for help or any ideas on how to write following parts of programing project so as to get good marks

-A description of the problem and the specifications for its solution
-A representation of the solution using a recognised design tool
-Your initial screen design/s on paper
-Text data used and results from testing the solution
-A self-evaluation sheet that evaluates your performance in the team

this part must be as of honesty, but also bullshit a bit.... its just to see if you're actually applying any thinking, first of all, a developer will not design a solution like, identical to Adobe Photoshop, and say, that there was a need for another one, that's not even correct, so you gotta judge on your program, why should it even be developed at first, and what's its distinct features in comparison to pre-existing applications of the same type, my SDD Major had met all these factors.
