Does anyone know the proper format/layout of a letter to the editor i.e. whose addresses do you include and where do you put them? How do you sign off?...name, suburb?

...this is for an english assessment.

yes yes yes...i have a damm sheet that contains lots of info on how to write a letter to the editor, ill scan it asap, if i can find it that is.
