Do they give out conceded passes to students or do stuents need to request them if they fail the course?

The PC is given at the discretion of the school. They will look at your other subjects' marks, and if they are all well and you've only just failed your subject - they might give you a PC.
I don't know of any application process for a PC.

i think they just give PC to you. I know for school of chemistry, if you score between 46 to 49, you can get PC.
If you score 49, your WAM has to be 52.
If you score 46, your WAM has to be 55.
And 48 to 47 is somewhere in between.
You can have a maximum of 18 uoc PC in for the whole your program.
If you score 49, your WAM has to be 52.
