Hi all.
I've been finalising my PREP application and I am confused with one specific area.

My year advisor says that you need to also submit the "supplementary form a" to the UAC, yet I cannot find any information about this on the UAC website nor CSUs.

Can anyone please tell me exactly what forms/applications need to be submitted to complete a PREP form?

It will be very much appreciated.

i think the supplementry forms are in the front of your UAC guide... if not just call UAC, im sure they can help you out.. i mean thats what theyre there for!

i rang uac last year cose it all seemed mumbo jombo and they told me what to do

Just follow the tick boxes on the back of the form...

I got my application today, and I have already filled it in, now just got to hand it in to school admin so they can get the principle to fill in his sections and attach my trial report.
