Questionnaire - Both (Open questions provide qualitative, closed q's give quantitative)
Focus groups - Qualitative
Personal reflection - Qualitative
Participant and Non participant observation - Qualitative

Quantitative mean you can gives numerical results and you can produce graphs etc from it

Think of a survey as a methodology that utilises various other methodologies to accumulate data on a particular subject of interest. For example say i was interested i teenage depression, i could conduct a survey on teenage depression by using a questionnaire, case study, interview and statistical analysis.

whats the difference between a survey and a questionnaire ?
A survey is essentially anything that uses a representative sample making generalisations upon a larger population. It is broader than a methodology and can take the form of an interview, questionnaire etc.

So a questionnaire is a written type of survey. In everyday language survey and questionnaire tend to be used interchanegably but technically they aren't the same thing necessarily.
