just some feedback on your questionnaire. overall it's pretty well designed, but it could be clarified in some areas- like by negative experiences, do you mean negative expereicnes of the current religion or of other religions?
q4- maybe you could have an option for the participant "other"...so they can list their beliefs. need to include buddhism. it's spelled "judaism"
q10- parents might have different beliefs to each other
q17- responses need to be clarified
q19- do you mean relevant?

sorry if i seem overly critical! it's cos i'm distributing questionnaires for research i'm doing at the moment, and i know how difficult it is to extract results if the original questions don't make sense, and you can't go back and change the wording,
